Case study performed by: Prof. Dr. Harikrishna K.R. Nair; Nurse Angeline; Wound Care Clinic HKL, Malaysia.

A 72-year-old male patient, employed as a security officer with limited financial support and low income, presented with a right diabetic foot ulcer (DFU) at the plantar medial midfoot aspect. The patient has a history of Diabetic Mellitus for the past 28 years, managed with S/C Actrapid 8 units TDS and S/C Lantus 13 units ON, with moderate glycemic and blood pressure control (11 mmol/L and 130/80 mmHg). He also has a history of Hypertension for the past 15 years. The ulcer developed following contact with a hot motor exhaust pipe, resulting in a blister and subsequent wound.

Method: L-Mesitran Tulle was applied to the wound bed and covered with a secondary dressing. The wound was cleansed regularly with sterile water, while an offloading pad was used to reduce pressure on the affected area. Dressing changes were performed according to the patient’s wound care regimen.

Results: After 50 days of treatment, the wound size decreased from 7.5 × 3.5 cm to 5.5 × 2.5 cm. The wound bed showed 50% granulation tissue and 50% epithelialisation tissue. Pain and malodour resolved completely, while the surrounding skin demonstrated reduced signs of maceration. The patient reported no pain at follow-up.

Discussion: The antimicrobial effects of MGH rely mainly on the acidic pH, the high osmotic pressure due to the high sugar content, and the release of hydrogen peroxide after the enzymatic breakdown of sugar(1, 2). In addition to its antimicrobial activities, MGH also promotes wound healing by keeping the wound moist, stimulating autolytic debridement, and exerting anti-inflammatory and anti-oxidative effects.

Conclusion: The use of MGH has been clinically effective and cost-efficient in managing this diabetic foot ulcer. These MGH-based products provide a viable alternative to more invasive interventions and can be easily applied, even in a home care setting, which is crucial for patients with limited resources. The reduction in wound size and pain indicates a positive outcome, ultimately improving the patient’s quality of life.
